The Case Management Department offers assistance to inpatients who require help with their discharge plans or whose living arrangement may interfere with their care or recovery. The Case Management department is made up of nurses, social workers, and discharge planners.

CVMC’s case management team helps customers find high-quality doctors. We do so by also providing patients with in-network discounts at the same time. Our case managers coordinate medical care for pre-planned procedures, recommend treatment plans, and keep tabs on ongoing medical care.
