Fall 2025 Safety Grade recognizes national commitment to patient safety and high-quality care
CHINO, Calif., November 13, 2025 – Chino Valley Medical Center, a member of Prime Healthcare, has earned an “A” Hospital Safety Grade from The Leapfrog Group, an independent national nonprofit watchdog dedicated to improving patient safety and healthcare quality. Leapfrog assigns single-letter grades to hospitals across the country based on their performance in preventing medical errors, injuries, accidents, and infections.
This recognition reaffirms Chino Valley Medical Center’s strong and ongoing commitment to maintaining a safe, high-reliability environment for patients.

The Leapfrog Hospital Safety Grade is the only national hospital ratings program that focuses exclusively on preventable medical errors, infections, and injuries—serious events that contribute to more than 500 patient deaths per day in the United States. The grading system is peer-reviewed, transparent, and updated twice annually in the spring and fall.
Chino Valley Medical Center continues to advance safety through ongoing initiatives such as enhanced infection-prevention protocols, multidisciplinary rounding, medication-safety practices, and continuous clinical education. These efforts support a culture of accountability, high performance, and patient-centered care.
